Main Duties
Deliver care to an allocated caseload of adults experiencing or recovering from mental health difficulties
Provide direct support and advice to service users and carers within the recovery framework
Work as part of a multi-disciplinary team and in partnership with internal and external stakeholders
Conduct assessments, care planning, and coordination of care from a social perspective
Deliver recovery-focused care prioritising choice, control, independence, and safety
Maintain continuous professional development and adhere to Social Work England standards
Undertake AMHP training if not already qualified, or contribute to the Trust’s AMHP rota if qualified
Key Responsibilities
Social Care & Safeguarding
Champion social care, legislation, and statutory guidance
Lead on adult safeguarding practice and enquiries
Conduct carer assessments and identify contingency plans
Ensure commissioned services allow choice, independence, and appropriate community support
Service Development & Improvement
Implement policy changes and identify areas for service improvement
Undertake audits, surveys, and evaluations to inform service development
Support newly qualified social workers through supervision and ASYE programme
Communication & Relationship Management
Communicate complex and sensitive information to service users, carers, and stakeholders
Build effective relationships within multi-disciplinary teams and across the Trust
Prepare reports and maintain accurate records using electronic systems
Clinical Responsibility
Deliver integrated assessment, care planning, and management for service users with complex needs
Exercise professional autonomy while receiving appropriate support and supervision
Leadership & Management
Provide guidance, supervision, and mentorship to junior staff
Contribute to team and countywide rotas, including AMHP duties
Promote high standards, team wellbeing, and adherence to Trust values
Financial Responsibility
Authorise care packages and handle service user resources according to legislation and Trust policies
Person Specification
Essential Qualifications & Experience
Professional Social Work qualification
Degree-level education
Registration with Social Work England
Post-qualification experience (minimum 2 years) in community-based social work
Safeguarding and relevant post-qualifying training
Experience in assessments, care planning, personalisation, and partnership working
Desirable Qualifications & Experience
AMHP or Best Interests Assessor training
Experience in large-scale mental health programmes and NHS systems
Skills & Competencies
Excellent verbal and written communication
Analytical and judgement skills for complex service user information
Organisational and planning skills under pressure

Ability to manage sensitive and contentious information
Other Requirements
Ability to travel independently across Hertfordshire sites
DBS check required
