Job Summary

Main Duties

  • Deliver care to an allocated caseload of adults experiencing or recovering from mental health difficulties

  • Provide direct support and advice to service users and carers within the recovery framework

  • Work as part of a multi-disciplinary team and in partnership with internal and external stakeholders

  • Conduct assessments, care planning, and coordination of care from a social perspective

  • Deliver recovery-focused care prioritising choice, control, independence, and safety

  • Maintain continuous professional development and adhere to Social Work England standards

  • Undertake AMHP training if not already qualified, or contribute to the Trust’s AMHP rota if qualified

Key Responsibilities

Social Care & Safeguarding

  • Champion social care, legislation, and statutory guidance

  • Lead on adult safeguarding practice and enquiries

  • Conduct carer assessments and identify contingency plans

  • Ensure commissioned services allow choice, independence, and appropriate community support

Service Development & Improvement

  • Implement policy changes and identify areas for service improvement

  • Undertake audits, surveys, and evaluations to inform service development

  • Support newly qualified social workers through supervision and ASYE programme

Communication & Relationship Management

  • Communicate complex and sensitive information to service users, carers, and stakeholders

  • Build effective relationships within multi-disciplinary teams and across the Trust

  • Prepare reports and maintain accurate records using electronic systems

Clinical Responsibility

  • Deliver integrated assessment, care planning, and management for service users with complex needs

  • Exercise professional autonomy while receiving appropriate support and supervision

Leadership & Management

  • Provide guidance, supervision, and mentorship to junior staff

  • Contribute to team and countywide rotas, including AMHP duties

  • Promote high standards, team wellbeing, and adherence to Trust values

Financial Responsibility

  • Authorise care packages and handle service user resources according to legislation and Trust policies

Person Specification

Essential Qualifications & Experience

  • Professional Social Work qualification

  • Degree-level education

  • Registration with Social Work England

  • Post-qualification experience (minimum 2 years) in community-based social work

  • Safeguarding and relevant post-qualifying training

  • Experience in assessments, care planning, personalisation, and partnership working

Desirable Qualifications & Experience

  • AMHP or Best Interests Assessor training

  • Experience in large-scale mental health programmes and NHS systems

Skills & Competencies

  • Excellent verbal and written communication

  • Analytical and judgement skills for complex service user information

  • Organisational and planning skills under pressure

  • Commitment to equality, diversity, and inclusion

  • Ability to manage sensitive and contentious information

Other Requirements

  • Ability to travel independently across Hertfordshire sites

  • DBS check required

  • Skilled Worker sponsorship available for eligible applicants
