Role & Responsibilities

• Support individuals transitioning from hospital to home, helping them regain independence

• Visit service users in their own homes to provide care as outlined in personalised care plans

• Assist with personal care, administering medication, monitoring vital signs, exercise, and domestic tasks

• Liaise with medical professionals such as district nurses and pharmacies

• Help service users rehabilitate, maximize their potential, and lead more independent lives

Working Hours & Pay

• Shifts: 7:00am–9:30pm

• Pay: £13.50–£15.50 per hour, depending on experience and suitability for a junior or senior role

Qualifications & Requirements

• Valid driving licence and access to own vehicle

• Experience in creating and implementing care plans

• Good communication skills in English

• Proficiency in IT for documentation and reporting

• Ability to provide compassionate home care

• Familiarity with care home environments and protocols
