Job Summary

An exciting opportunity is available to join the Older Persons and Physical Disability Teams as a Level 2 Registered Social Worker based in the Staffordshire Moorlands.

The service supports people of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

The role requires a highly skilled professional with excellent communication and organisational skills with an excellent working knowledge of the Care Act 2014 and Mental Capacity Act 2005.

Main Duties and Responsibilities

As a Level 2 Social Worker, a flexible approach is required with the ability to build strong working relationships with multi-disciplinary teams and partner agencies. The role requires an experienced practitioner with in-depth knowledge of safeguarding and complex case management.

Key responsibilities include:

  • Playing a key role in supporting delivery of the Personalisation and Inclusion Pathways for Adults and Carers, ensuring legal literacy, strength and asset-based practice, personalisation, and social inclusion are effectively embedded

  • Carrying out holistic needs-led assessments, complex case work, care and support planning, reviews, and initiating multi-disciplinary assessments where appropriate

  • Demonstrating comprehensive knowledge of services available to promote independence in home environments

  • Providing strength-based practice and forging community links to deliver outcome-focused, person-centred services within an integrated care system

Working Model and Professional Development

The team currently operates using a hybrid working model with remote working options available. Flexible working arrangements are offered in line with service needs, supporting reflection time, autonomy, wellbeing, and work-life balance.

Upon appointment, enrolment into the SWLA will occur and alignment to the appropriate British Association of Social Work Professional Capabilities Framework will be completed. The relevant SWLA stage (Stage 1 ASYE, Stage 2 Social Worker, or Stage 3 Advanced Social Worker) will be agreed based on experience and CPD history.

The post holder will be required to take responsibility for ongoing Continuous Professional Development.

Qualifications and Training

Essential

  • Professional Social Work qualification (DipSW, CQSW, CSS, BA, MA or foreign qualification certified by the Professional Social Work regulatory body)

  • Current registration with the Professional Social Work regulatory body

  • Holding or undertaking Approved Mental Health Professional and/or Best Interest Assessor status as per the SWLA Career Pathway

  • Undertaking and achieving Work Based Supervisor and Practice Educator status as per the SWLA Career Pathway

  • Good IT skills including word processing, spreadsheets, email and PowerPoint

  • Ongoing commitment to professional development

  • Clear understanding of equal opportunities and anti-discriminatory practice

Desirable

  • Existing accreditation relating to Practice Educator or Work Based Supervisor

Experience Requirements

Essential

  • Significant post-qualification experience working with adults with needs related to older age, physical health, or mental health and carers

Desirable

  • Experience across other social work settings including Children and Families, Learning Disabilities, or Older Adults

  • Experience within service or facility development roles

Disclosure and Background Checks

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. Disclosure and Barring Service checks will be required to review previous criminal convictions.

From 6 April 2017, Skilled Worker applicants applying for UK entry clearance must provide criminal record certificates from countries resided in for 12 months or more during the past 10 years. Adult dependants over 18 years are also subject to this requirement.
