Job Summary

North Bristol NHS Trust is offering an exciting opportunity for a Social Worker to join the Renal & Transplant department at Southmead Hospital. This temporary role is for a minimum of 18 months, with a potential extension depending on approval.

The postholder will work with kidney patients, providing person-centred care that focuses on strengths to reduce the psychological impact of chronic kidney disease. This includes social work assessments and interventions for patients and their families across a large geographical area, including a designated renal ward, outpatient clinics, transplant clinics, and dialysis units.

Applicants must hold a recognised Social Work qualification (BSc, MSW, DipSW, CQSW, or equivalent) and be registered with Social Work England. Experience in acute health settings is desirable.

The role will also scope and design a dedicated social worker service to support renal patients, promote adjustment, and improve engagement with medical treatments recommended by the renal medical team.

Main Duties

  • Provide expert social care advice and guidance within the adult renal community

  • Contribute to complex assessments and reviews for high-risk patients

  • Assess patients’ health and social care needs, including interpersonal, social, legal, or environmental complexities

  • Identify and highlight the need for commissioned services to ensure patient choice

  • Contribute to risk assessments and promote safeguarding practices in line with policy

  • Advise colleagues on best practices for assessing carers’ needs and contingency planning

  • Identify recovery-focused community services and ensure colleagues are aware of best practices

  • Provide guidance in complex crisis situations as requested by medical staff and the multidisciplinary team

  • Scope the benefits of a dedicated renal social worker in liaison with senior staff

Person Specification

Training, Knowledge & Experience

Essential:

  • Undergraduate degree in Social Work

  • Knowledge of legal framework relevant to adult nephrology

  • Experience of multi-disciplinary team working

  • Sound knowledge of relevant specialty and transferrable skills

  • Excellent interpersonal and communication skills

  • Ability to remain professional in highly emotionally charged scenarios

Desirable:

  • Experience working in adult acute hospitals

  • Recognised management or leadership qualification

  • Experience with adults living with chronic health conditions

Knowledge & Experience

Essential:

  • Legislation and national renal guidance pertinent to the role

Desirable:

  • Experience working in acute hospital settings
