Job Summary
Cheshire and Wirral Partnership NHS Foundation Trust (CWP) is recruiting a qualified Social Worker to join its Integrated Services for adults with disabilities at the Millennium Centre in Leasowe, Wirral.
The team works with people with complex needs and their carers, supporting them to achieve positive outcomes. This service focuses on health and social care integration and encourages innovative approaches to care.
Key responsibilities include:
Conducting Care Act assessments.
Developing personalised support plans.
Completing mental capacity assessments.
Leading safeguarding enquiries.
A strong understanding of asset- and strength-based approaches to assessment and support planning is essential. Career progression and training opportunities are provided.

Main Duties
Practice as a person-centred Social Worker.
Carry out Care Act assessments and develop personalised support plans.
Lead safeguarding enquiries and complete mental capacity assessments.
Apply asset- and strength-based approaches to assessment and support.
Register and maintain status with Social Work England (SWE).
Participate in CWP’s career progression and training programmes.

Person Specification
Qualifications
Essential: Social Work degree or DipSW, registered with SWE, CPD in line with PCF, willingness to undertake Practice Educator Award.
Desirable: Post-qualifying/vocational training courses.
Knowledge & Expertise
Essential: Knowledge of relevant legislation (e.g., Mental Health Act 1983, Mental Capacity Act 2005, Disability Discrimination Act 1995, Care Act 2014, Safeguarding).
Strong verbal and written communication skills, organisational skills, commitment to anti-discriminatory practice, ability to work with diverse individuals, families, carers, and partner agencies.
IT literacy, including Microsoft Office.
Desirable: Emotional intelligence, resilience, experience in multiagency teams.
Experience
Essential: Working with carers and vulnerable individuals/families, arranging support plans or packages of care, experience with partner agencies.
Desirable: Experience in Local Authority settings, multiagency work, supporting families with varied needs.

DBS check required. Costs deducted from salary; enrolment in DBS Update Service encouraged.
