Job Summary
As a Social Worker, the role involves working as part of a multi-disciplinary team to provide high-quality social care services to adults of all ages experiencing mental ill-health.
The core focus includes assessment, care planning, and coordinating care from a social perspective. The role is recovery-focused, supporting service users and carers based on choice, control, independence, and safety. The Social Worker also acts as a champion of social care and safeguarding within the local area.

Main Duties of the Job
The Social Worker will:
- Promote social care practice with strong understanding of legislation and statutory guidance
- Lead on adult safeguarding and undertake safeguarding enquiries
- Contribute to care plans protecting adults and children from abuse
- Carry out comprehensive assessments and reviews for individuals with complex needs
- Apply the Care Act 2014 eligibility criteria in assessments
- Provide information, advice, and signposting for service users not meeting eligibility
- Support commissioning of health and social care services, including direct payments
- Ensure services promote choice, independence, and community living
- Conduct regular risk assessments to ensure safety of vulnerable individuals
- Work in partnership with service users and carers to improve services

- Managing a caseload of adults with varied mental health and social care needs
- Promoting equality, diversity, and challenging discrimination
- Supporting social inclusion and addressing disadvantage
- Maintaining continuous professional development in line with Social Work England standards
- Preparing for or contributing to the Approved Mental Health Professional (AMHP) rota if applicable
- Working collaboratively with multidisciplinary teams and external partners
Person Specification
Knowledge, Training & Experience
Essential:
- Social Work qualification
- Degree level education
- Social Work England registration
- Training in outcome-focused assessments and personalisation
- Post-qualifying social work training
- Safeguarding training and experience
Desirable:
- Approved Mental Health Professional (AMHP) training
- Best Interests Assessor training
Experience and Knowledge
Essential:
- Post-qualification experience in social work
- Knowledge of relevant legislation, theory, and policy
- Experience in assessments and care planning
- Understanding of personal budgets and direct payments
- Experience working in multidisciplinary teams
- Experience working collaboratively with service users and carers
Desirable:
- Experience as AMHP or Best Interests Assessor
