Main Duties
What you will do:
- Work closely with adults and families to provide tailored support and care planning using a strengths-based and person-centred approach
- Champion independence, dignity, and wellbeing in every interaction
- Collaborate with health professionals, community partners, and colleagues to deliver joined-up services
- Bring compassion, resilience, and creativity to complex situations
- Support workforce development through supervision of up to 4 Community Care Officers
Job Responsibilities
As part of the wider council objectives, responsibility includes delivering statutory social work support to adults with differing needs to promote independent living and appropriate care and support.
Key responsibilities include:
- Managing a varied and complex caseload with appropriate supervision
- Conducting Care Act 2014 assessments using a strengths-based approach
- Identifying outcomes, independence opportunities, and support solutions
- Contributing to safeguarding enquiries in line with council policies
- Deputising for Advanced Practitioners when required
- Representing senior staff in multi-disciplinary meetings
- Supervising up to four non-registered practitioners
- Providing mentoring and supporting students or newly qualified social workers
- Working collaboratively across health and care sectors
- Undertaking travel outside the county when required

Essential
- Professionally qualified Social Worker
- Current Social Work England (SWE) registration
Experience
Essential
- Minimum 2 years post-qualification experience, OR
- 1 year post-qualification (ASYE completed) plus 3 years relevant experience in a complex health and social care setting
Additional Requirements
- Subject to Disclosure and Barring Service (DBS) check
- Skilled Worker visa sponsorship applications are considered
- Overseas applicants must provide criminal record certificates (where applicable)
- UK professional registration is required
