Job Summary

Due to career progression, there is a vacancy within Integrated Services for adults with disabilities based at the Millennium Centre in Leasowe, Wirral.

The team works with people with complex needs to support them and their carers in achieving the outcomes they need. Cheshire and Wirral Partnership NHS Foundation Trust is at the forefront of health and social care integration, supporting innovative ways of working together.

This role requires a person-centred practitioner with experience in Care Act assessments, personalised support planning, mental capacity assessments, and safeguarding enquiries. A strong understanding of asset-based and strength-based approaches is essential.

Qualified social workers must be registered with Social Work England (SWE). Applications are also welcomed from newly qualified staff or final-year social work students, with salary differing from Band 6 in these cases.

Main Duties

  • Carry out Care Act assessments
  • Develop personalised support plans
  • Complete mental capacity assessments
  • Lead safeguarding enquiries
  • Apply asset-based and strength-based approaches to assessment and planning

Person Specification

Qualifications

Essential

  • Social Work qualification (Degree in Social Work, DipSW or equivalent)
  • Registered with Social Work England
  • Evidence of continuing professional development in line with PCF
  • Willingness to undertake practice educator award

Desirable

  • Relevant post-qualifying or vocational training

Knowledge and Expertise

Essential

  • Knowledge of relevant legislation (Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Safeguarding, Disability Discrimination Act 1995)
  • Understanding of confidentiality and data protection
  • Strong communication skills (written and verbal)

  • Ability to work with individuals, families, carers, and professionals
  • Ability to adapt across teams based on service needs

Additional Requirements
